If you wish to register with this practice you will need to complete a form that the receptionist will supply. If you like, there is an online version of this form which may be downloaded here. Complete the following forms along with the Registration Form

THE PRACTICE NEEDS ALL 3 FORMS TO REGISTER A NEW PATIENT
If you have your NHS number it would be handy to bring to complete registration at the practice, and a form of Identification (preferably photo ID and something to confirm your address if possible). This would also help with online registration. There is no regulatory requirement to prove identity , address or immigration status to register at a GP surgery.
You will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this questionnaire which will need to brought along with your registration and summary care record forms.
All patients will be allocated a named GP who is responsible for patients' overall care at the practice. If a patient has a preference as to which GP that it is, the practice will make reasonable efforts to accommodate this request.
Once registered you will be offered a chance to have a health check with our practice nurse, where you will be able to discuss any ongoing medical problems.
The nurse will advise you on which is the best way to progress, either by seeing a GP of your choice or booking an appointment in one of our clinics. All patients can choose which practitioner they wish to see.
